AGREEMENT FOR RECORDING DAVID KOHLHAGEN, LPC BY ANY NON-PROFIT PUBLIC
SERVICE ORGANIZATION, PERSON, OR ENTITY, AND BY ANY PERSON OPERATING RECORDING EQUIPMENT
ON BEHALF OF ANY NON-PROFIT PUBLIC SERVICE ORGANIZATION, PERSON, OR ENTITY
It is my pleasure and my intention to share my knowledge and expertise with your
non-profit group or organization. I make a living as a professional counselor in
private practice, and I have personally created the material that I present to groups.
Much of the content is published on my website, and it is protected by copyright
and intellectual property law. I invite and encourage you to audio and/or video
record my presentation to your group; it can be highly beneficial for both of us.
Therefore, I hereby agree to permit my presentations to be recorded by any member
of your group or by any outside resource of your choosing, for the educational and
informational use of your non-profit group, upon the following conditions:
That I receive a playable copy of any resulting recording at no cost to myself;
That you agree to use the recordings solely for the education of the members of
your group and the public, or to promote your video filming business;
That your organization or entity and videographer agrees not to charge money or any other consideration for copies of the recording;
That you agree to notify me of any intended publication of the recordings prior
to their publication (for example, uploading to YouTube);
That I reserve the right to prohibit such recordings from being distributed or published
if they represent me or my work in any way that I deem professionally unsuitable
or inaccurate;
That I am acknowledged in writing on the product, in the film or audio recording,
and in any advertising of the product as the owner of the copyright of the recording,

That I am identified in writing on the product, in the film or audio recording,
and on any advertising about the product as the author of the recording, together
